Failure Analysis

In a complex system with serious consequences of failure, what is the most efficient solution to ensure its continued operation?

Helping our client understand and improve a system’s reliability.

When performing failure analysis, SMA takes multiple factors into consideration: the various ways a system can fail and the overall likelihood and consequences of those failures; the system’s built-in redundancy; the availability of parts and the time needed to source them; and finally, the most effective methods of preventative maintenance and the resources required to achieve it. SMA performs a comprehensive failure and reliability analysis of the system to minimize risk of failure and to mitigate those risks that exist.

The methodologies we use include Failure Mode, Effect, and Criticality Analysis (FMECA); Hazard and Operability (HAZOP) study; and Reliability Analysis, as well as incorporating simulation modeling to examine the operation of complex systems in a transparent fashion that includes the factor of randomness.
